Admissions to Foundations for Living

Located in Mansfield, Ohio, Foundations for Living is a secure, residential treatment center for males, ages 10-15, and females, ages 11-17, with a minimum IQ of 65. Program tracks include a short-term assessment option and a long-term treatment program.

Long-Term Treatment Admission Criteria:

Boys

  • Ages 10-15 in need of intensive mental health and/or substance-dependency treatment
  • Minimum IQ of 65
  • No current treatment needs for sex offenses
  • School eligible (no high school graduates/GED)

Girls

  • Ages 11–17 in need of intensive mental health and/or substance-related disorder treatment
  • Minimum IQ of 65
  • No current treatment needs for sex offenses
  • School eligible (no high school graduates/GED)

Short-Term Admission Criteria:

  • No more than one admission to an emergency/acute psychiatric unit within the last three months
  • Is not being placed due to a letter of removal from another residential facility or is being transferred from another residential facility
  • IQ of 70 or higher and a reading level at the seventh grade level or higher
  • The youth ages 10-15 for males, or ages 11-17 for females
  • May not be an adjudicated sex offender or have a GSI charge
